Challenge

 

Care is moving closer and closer to home. With investments in digital healthcare increasing, technologies advancing to accommodate this growing need, and more companies emerging to deliver care where patients want it most, the healthcare system needed a leader to show them the way. Recognizing this growing need that hospitals and medical offices are facing, the American Medical Association needed to not only define the Health at Home moment, but also create a framework for successful integration of delivering care at home.

 
 

Insight

 

The healthcare system is at an inflection point as it considers new and evolved models of care that converge with the experience patients have come to expect. A framework that defines Health at Home is the foundation for transforming the way we think about caring for patients.

 
 

Solution

 

Based on findings from secondary research, subject matter expert input and interviews with leading healthcare organizations, we collaborated with the American Medical Association to develop a Health at Home framework that represents the types of services that are being or can be delivered in the home setting and the infrastructure necessary for doing so successfully.
